Home Care Packages

Our Home Care Packages ensure you have a comprehensive Care Plan and access to the services you need to remain living well at home. Services available to you in your Home Care Package include:

• personal care

• support services

• allied health and clinical care

• transport

Our Home Care Packages provide you or a loved one with the support to live independently at home longer. And that’s a great place to be. The four levels of Home Care Packages include:

• Level 1 Basic Care: Basic-level care is suited to those clients that require short-term care. Clients on level one care, where the HCP budget is honoured, can access additional episodic or short-term allied health, therapy or nursing services.

• Low-level Care: Low-level care is suited to those that require regular assistance and support. Clients on low-level care plans usually require 3-4 hours of help per week.

• Level 3 Intermediate Care: Intermediate care is suited to those that require frequent assistance and support. These clients typically require care 6-7 days a week.

• Level 4 High-level Care: High-level care is offered to those that require full-time assistance and support, or who may have complex care needs.
