Clinical Care Coordination

Delivers client-centered services to adults experiencing severe and complex mental health issues who are able to be managed in primary health care. Supports the coordination of a client's multi-disciplinary team as well as support to gain access to a range of additional service offerings they may need including psychosocial supports and psychological therapies.
