COPD Supported Discharge

Clinical care coordination and self-management support to individuals with chronic obstructive pulmonary disease (COPD), who are non-oxygen dependent, following a COPD related hospital admission.

The service supports individuals immediately post-discharge and then facilitates connection to general practice and referral to Asthma WA for ongoing community based support, with the aim of establishing more effective community based care and reduced hospital admissions. Patients are referred into the service by metropolitan tertiary hospitals on discharge.
