Offers free/low cost support for adults living in the City of Cockburn (or willing to travel) to improve their healthy eating or physical activity behaviours and achieve or maintain a healthy weight. The service provides ongoing support through two-monthly health consults and connects clients to services tailored to their healthy lifestyle goals.
